Job Description:

Agape Care Group is seeking a dedicated Pediatric Social Worker to join our compassionate team, providing special care in hospice and palliative settings across the Southeast. This part-time role focuses on supporting children and their families through challenging times, ensuring they receive the utmost care, comfort, and dignity. As a member of Agape Care Group, youll work in a supportive environment aimed at offering peace and quality of life to our youngest patients and their loved ones. Assess the psychosocial status of patients and families/caregivers related to the patient’s terminal illness and environment. Carry out social evaluations, including family dynamics, caregiver abilities, communication patterns, high risks for suicide, neglect or abuse and plan intervention based on evaluation findings. Counsel patient and family/caregivers as needed in relationship to stress, and other identified coping difficulties. Maintain clinical records on all patients referred to social work. Provide information and referral services for organization patients and families/caregivers regarding practical and environmental needs. Provide information to patients and families on Medicare, Medicaid, and other financial assistance programs available to the pediatric patient. Participate in the development of the individualized plan of care, involving the patient and family, and attends regularly scheduled interdisciplinary group meetings. Assist physician and other team members in understanding significant social and emotional factors related to health problems and death/dying issues. Strong interpersonal, organizational, and critical thinking skills. Excellent communication skills, both verbal and written. Ability to work effectively as a part of a team and independently. Knowledge of and ability to navigate healthcare systems, community resources, and internet-based research for client benefit. Proficiency with healthcare technology and Electronic Medical Records (EMR). Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Current, unrestricted Social Work License in South Carolina is required. BLS certification preferred.
