Work schedule: 2 - 3 weekdays and every other weekend.**POSITION SUMMARY: - -CVS Health is seeking a Nurse Practitioner (NP) to join our Primary Care team. In this capacity, you will work in collaboration with a dedicated clinical care team to provide evidenced-based care to a panel of patients. The clinical care team will work with the provider daily to drive better outcomes for entire panel of patients. -This will include accurate assessment, diagnosis, treatment, management of health problems, health counseling, and disposition planning. This role will report to the practice manager.PRIMARY DUTIES AND RESPONSIBILITIES: Responsibilities include but are not limited toPatient-Centered Quality and Safety

• Uses knowledge of etiologies, risk factors, pathophysiology, presentation, and epidemiology of medical conditions relevant to primary, acute, chronic, and transitional to provide care to patients
• Performs history and physical assessments of patients that are accurate, complete, concise, relevant, and appropriate for age, gender, and clinical symptoms.
• Provides patient counseling; inclusive of pregnancy prevention, STI Prevention/safer sex practices, contraceptive care counseling and medication management
• Counsels and educates patients and families on health maintenance and respond to patient care inquiries
• Evaluates and treats patients' medical needs with safe and high-quality treatment
• Documents all patient care within an electronic health record in accordance with current policies and procedures
• Works as a member of the clinical care team providing care and coordination of our patients with internal and external colleagues ensuring the highest level of care is always provided for all patients
  • Provides patient-centered care that is compassionate, appropriate, and effective for the promotion of health, prevention of illness and treatment of disease
    • Refers to appropriate specialists and patient care components as appropriate
    • Maintain patient confidentiality in accordance with PHI and HIPAA standardsCommunication and Interdisciplinary Collaboration
      • Collaborates with clinical care team to ensure the delivery of high quality, patient centric, longitudinal care
      • Participates in and in some cases leads clinical care team huddles

      • On call rotation shared with providers in market for after hours patient clinical needs or shift coverage

            • Continuing education, including what is required to maintain employment at CVS Health/ Primary Care, is the responsibility of the providerEDUCATION: - Completion of master's degree level Family Nurse Practitioner with current National Board Certification and State of Employment license to practice in the Advanced Practice role required.Required Experience:
              • A minimum of 1 year experience as a Nurse Practitioner is required. -Experience in Family medicine or primary care practice preferred. -
              • Internal applicants with less than 1 year experience are eligible with successful completion of the CVS Minute Clinic Primary Care Training Program
              • Active Advance Practice Registered Nurse, Nurse Practitioner license in good standing
              • Current National Board Certification in Family PracticeADDITIONAL REQUIRED SKILLS AND ABILITIES: -
