About Us Our organization has been a pioneer in establishing the
first national in-home clinical network, and we currently operate
as an independent provider of extensive health assessments for
individuals nationwide. We are dedicated to delivering exceptional,
personalized care that enhances health, reduces costs, and empowers
individuals to lead fulfilling lives. With a strong foundation in
clinical assessment and care management, our nationwide network of
clinicians transcends traditional barriers to healthcare by meeting
members in their own environments. We assist older adults and other
at-risk populations in achieving better care quality, improved
health outcomes, and the identification of chronic conditions that
might otherwise remain undetected. Our job openings enable you to
utilize your skills and compassion to make a meaningful difference
in the health and well-being of others. Join our team and enjoy
competitive compensation along with flexible scheduling while
contributing positively to the community! Responsibilities About
the Role During visits that may last up to one hour, our providers
assess and observe a member’s current health status, medical
history, medication adherence, social circumstances, and other
potential risks. This approach provides invaluable insights into a
member’s overall health that are often missed during standard
office visits. The Matrix Comprehensive Health Assessment (CHA)
aims to enhance care quality and allows us to address multiple care
gaps in a single visit. Our providers are responsible for:
Conducting assessments for adults and geriatrics, which include
reviewing medical history, diagnosing and treating conditions,
providing health education, making physician referrals, and
ensuring thorough documentation in line with our guidelines and
protocols. Administering and ordering necessary point-of-care and
lab-based tests. Collaborating with physicians, case managers,
social workers, family members, key caregivers, and other medical
personnel as needed. Working alongside Primary Care Physicians
(PCPs) to educate patients and follow up as required. Delivering
services at the member’s residence, whether in their homes or in
Skilled Nursing Facilities. Benefits of Working with Us Flexibility
– You can schedule visits according to YOUR availability.
Assessments are conducted from Sunday to Saturday between 8 am and
7 pm, allowing you to work during the day, evening, or weekend
based on your preferences. Competitive Pay – Your earnings will be
based on the number of assessments you complete. You can increase
your income by being more available and traveling to see members
outside your home area. Those averaging 20 to 100 visits per month
can earn between $1,800 and $11,500 , depending on the per
assessment rate of $90 to $115 based on your state of residence.
Benefits – Including mileage reimbursement, a 401(k) plan, employee
referral bonuses, and limited voluntary benefits. Education and
Support – We are committed to investing in YOU! Along with
comprehensive onboarding training, you will be assigned a
preceptor, receive personalized feedback, and have free access to
continuing education opportunities. Additional Licensure – If there
is a business need, we will assist you in obtaining additional
state licensure and credentialing in neighboring states or others
you may wish to visit while conducting health assessments.
Full-time Opportunities – Our PRN providers have the option to
transition into full-time roles supporting their home location or a
specific state or region, along with eligibility for a full-time
sign-on bonus. Qualifications Skills & Experience That Shine A
Master’s Degree or equivalent experience along with successful
