Job Description:
Agape Care Group is seeking a compassionate and dedicated Nurse
Practitioner to join our team, providing top-tier hospice and
palliative care in the Southeast. In this role, you will be crucial
in implementing our mission to enhance the dignity and comfort of
patients at a poignant time in their lives. Through personalized
care plans, you will address both the medical and emotional needs
of patients and their families, facilitating meaningful and
peaceful end-of-life experiences. Conduct comprehensive patient
assessments to determine medical and emotional needs. Develop and
implement personalized care plans in collaboration with a
multidisciplinary team. Manage symptom relief and prescribe
medications in accordance with patient health status. Provide
direct care and make house calls to assess and manage patient
conditions. Educate patients and their families on disease
processes, treatment, end-of-life care and other pertinent health
issues. Monitor and report changes in patient conditions, modifying
care plans as needed. Serve as a compassionate liaison between
patients, families, and other healthcare professionals. Strong
clinical skills in a hospice or palliative care setting Excellent
communication and interpersonal skills Ability to work
collaboratively in a multidisciplinary team Highly organized and
detail-oriented Empathy and emotional intelligence Medical

Active Nurse Practitioner license Certification in Hospice and
Palliative Care Nursing (CHPN) preferred
