Job Description:
At Agape Care Group, we are seeking a dedicated and
compassionate Registered Nurse to join our after-hours hospice
team. This position is vital in providing exceptional nursing care
to hospice patients during non-business hours, ensuring they
continue to receive utmost comfort and support in alignment with
our mission to enhance quality of life through the end stages of
life's journey. Ideal candidates will demonstrate superb
communication skills, emotional resilience, and a deep commitment
to end-of-life care. Provide direct patient care to hospice
patients during after-hours, weekends, and holidays. Assess patient
conditions during visits and adjust the care plan as necessary in
consultation with the attending physician. Administer medications,
wound care, and other personalized interventions as outlined in
patient's care plan. Educate and support patients' families on
end-of-life care and comfort measures. Coordinate with the hospice
team to ensure continuity of care and share updates regarding
patients' conditions and treatments. Document care in a timely and
accurate manner, adhering to all compliance and regulatory
requirements. Proficient in providing holistic patient care and
pain management. Adept at using electronic health record (EHR)
systems. Strong organizational and communication skills. Ability to
handle emotional situations with professionalism and empathy.
Medical Insurance Dental Insurance Vision Insurance 401k Life
Insurance Valid RN license in the state of practice. CPR
certification. Certified Hospice and Palliative Nurse (CHPN)
preferred but not required.
