Job Description:
This PRN Nurse Practitioner role involves conducting
comprehensive home risk assessments for at-risk populations,
including older adults and those in community settings in
Gallipolis, OH.
Ideal for new grads with at least 1 year of NP experience, holding
current licensure and board certification in a Matrix-approved
specialty.
Assessments last up to one hour, covering medical history, social
factors, and risks to improve care quality and address care
gaps.
Work collaboratively with physicians, case managers, and family
members across various venues, including home visits and skilled
facilities.
Flexible schedule options include days, evenings, nights, and
weekends; compensation ranges from $1,900 to $9,500 per month based
on assessments completed.
Must have valid driver's license, reliable transportation, and be
comfortable traveling. Benefits include mileage reimbursement,
401(k), and voluntary benefits.
Requires current RN and NP licensure, certification (BLS, ACLS),
computer skills, and excellent communication.
