  • NC or compact--License
  • 2+ years Home Health/Continuing Care
  • Experience with wound care, pleurx, infusion, venipuncture, case management, SOC
  • OASIS and HCHB experience
  • AHA BLS
  • Graduate from an accredited School of Nursing
  • BSN--requiredOther Details:
    • Performs Home Health visits, start of care, resumption of care assessments in the clients home setting.--
    • Works with a multidisciplinary team to provide patient centered care to clients.--
