Description: Coordinate clinical care of assigned home care
patients with lead social worker EDUCATIONAL QUALIFICATIONS:Current
NC license as a registered nurseBachelor of Science degree in
nursing requiredActive patient contact or completion of a nursing
refresher course within the past 3 yearsCPR certification KNOWLEDGE
AND SKILLS:Knowledge of and commitment to the hospice philosophy of
careAbility to work on an interdisciplinary teamAbility to work a
flexible scheduleWillingness to work with patients/ families
regardless of race, color, creed, age or sexual orientation. Must
respect their beliefs and philosophy with an attitude of openness
and receptivenessStrong communication skills Initial And
Ongoing:Complete Hospice volunteer/staff training programComplete
Hospice orientation programObtain 12 hours of continuing education
per year, including mandatory safety in-serviceAttend minimum of
six staff meetings per yearAttend assigned team meetings every two
weeksMaintain CPR certification DUTIES AND RESPONSIBILITIES:Assist
in orienting new team members, as directedParticipate in quality
improvement programAdhere to strict confidentialityAdhere to safety
policy and practicesComplete employee time record accurately per
policyCooperate with all government agencies and outside
auditorsAssist with updating policies and procedures, as
appropriateComplete annual TB screeningAssure all license,
certifications, continuing education information and any other
required documentation are submitted to human resources and kept
currentMaintain up-to-date documentation required and specified in
Hospice policy and procedurePerform other duties assigned by
supervisorPartner with lead social worker to assure quality
patient/family careAssure outpatient treatment services are
appropriate and coordinated with business services and social
workers and directorBe available daily to provide direction to team
assuring appropriate interventionsAssure team members have adequate
supplies, equipment and physical spaceMaintain current knowledge
and competency in providing care to terminally ill patients and
their familiesAssist with entering Hospice referrals into software
system, as neededCoordinate the determination of appropriateness of
referrals with hospice physiciansCoordinate patient assignment of
primary nurse and certified nursing assistantCommunicate with
on-call team members and patient care unit staff on regular
basisSupervise certified nursing assistant assignmentsAssure timely
and appropriate utilization review of patientsCoordinate pharmacy,
medical equipment and other contracted services and monitor
compliancyCoordinate delivery of care with primary team and
attending physicianBe available to receive telephone calls from
staff, physicians, health care facilities, and families to
coordinate services with primary teamPerform duties of primary
nurse, and patient care unit nurse, as neededCoordinate continuous
care servicesAssist with the maintenance of medical records
assuring accuracy, completeness, and compliance with all
regulations and standardsSupervise patient medical supplies
inventoryAssess outpatient services and assure appropriatenessMake
appropriate certified nursing assistant assignments via Cell Trak,
update assignments and make corrections, as neededCommunicate all
clinical information to director of nursing services and director
of high country servicesAssure 24-hour on-call services and be
available for on-call services, as needed services and marketing to
coordinate orientation for new nurses and certified nursing
assistantsAssure all nursing services staff promote the philosophy
of hospice as directed by the chief executive officerComplete
reports and other statistical data as requestedKeep abreast of all
regulations and communicate information to nurses and certified
nursing assistants PHYSICAL QUALIFICATIONS:Speech and hearing
skills necessary for telephone interaction and face to face
contactVisual acuity to read chart and physician's ordersAbility to
stand and walk for long periods of timeManual dexterity to
establish IV sites, manipulate tubing, and other patient care
suppliesWith assistance, ability to lift and support an average of
150 poundsAbility to sit as required to perform dutiesAbility to
pull/push, and bend frequentlyAbility to squat, kneel, climb, and
